What is Imiquimod Cream (Aldara)?
Imiquimod is a substance that stimulates your body’s immune system to produce substances. This substance helps to fight off different forms of skin damage.
When to treat with Imiquimod?
Treatment with Aldara is recommended for:
- Actinic keratosis (other names: keratosis actinica, solar keratosis, senile keratoses)
- Actinic keratosis on the lip (cheilitis actinica)
- Superficial forms of basal cell carcinoma (superficial or trunk type)
- Precancerous stage of squamous cell carcinoma (Morbus Bowen, Bowen’s disease)
- Genital warts (condylomas)
- Common warts (verruca vulgaris) and plantar warts (verruca plantaris).
Dosage of Aldara
The cream is packaged in sachets, each containing 250 mg of cream. One sachet contains enough cream for a skin area of 20 cm2. You use a new sachet each day. Opened sachets must be discarded.
When do you apply the cream?
Apply the cream before going to bed and then leave the treated skin undisturbed for about 8 hours.
How to use Imiquimod Cream?
- Wash your hands with soap and water first. Open a new sachet and squeeze some cream onto your fingertip.
- Apply enough Imiquimod cream to the area to be treated and 1 cm around the spot.
- Applying a thin layer of cream is sufficient. Applying too much cream can cause an increased reaction, so ‘more’ is not always ‘better’.
- If you have applied too much cream, you can wash it off with water and mild soap or wipe it off with a tissue.
- Wash your hands again with soap and water after applying the cream.
THE TREATMENT – SUMMARY
For Actinic Keratosis
For actinic keratosis, you apply the cream once a day. You do this 3 days a week (on Monday, Wednesday and Friday). You do this for 3 weeks. Then you can stop.
For basal cell carcinoma and Morbus Bowen, you apply the cream once a day. You do this 5 days a week (on Monday, Tuesday, Wednesday, Thursday and Friday). You do this for 6 weeks. Then you can stop.
For other indications
For all other indications, the dermatologist will discuss the treatment plan with you.
Important during use
- Do not cover the treated area or areas with bandages, gauze or plasters. If this is necessary, the doctor will discuss this with you.
- Make sure that the cream does not come into contact with the mucous membranes or eyes. Be careful when applying to areas around the eyes and mouth.
- When used for genital warts, it may be applied to mucous membranes – on prescription from the doctor.
- Do not use other skin products such as creams, lotions, cosmetics or medicines at the same time. If this is necessary, the doctor will discuss this with you.
- During the healing phase, the doctor may sometimes prescribe a disinfecting cream. For example, Chlorhexidine cream.
Avoid sunlight and other UV light (sunbed). Depending on the areas to be treated, it is advisable to wear a hat or clothing with long sleeves, for example.
What happens to the skin during treatment?
Imiquimod activates the immune system. This means that the sun-damaged or virus-infected cells that are not naturally recognized well enough by the immune system are recognized and cleared during this treatment.
At first, there is nothing to see right away. After a while, the areas where the rapidly dividing cells are located become damaged. The cells die.
What will the skin look like during treatment?
At first the skin is red, then blisters and flakes appear. Then deeper wounds and sometimes even ulcers develop. After stopping the application, the skin will recover.
Healthy skin cells grow from the edges and the depths of the wounds. So it gets worse first, then it gets better again. The skin is usually healed within 1 to 2 weeks.
Don’t worry if the skin breaks down after a while or if the skin feels itchy and painful. This is a sign that the cream is working. The redness, crusting and flakes are caused by the death of the abnormal cells. This is quite normal with this treatment. Just continue the treatment and do not stop it.
