Xarelto (Rivaroxaban), works by preventing the formation of blood clots after a certain alternating heartbeat or after hip or knee replacement surgery. It is also used to prevent and treat blood clots such as deep vein thrombosis (DVT) or pulmonary embolism (PE).

How it works?
Anticoagulants, also known as blood thinners, prevent blood clots from forming by slowing down the clotting process. Xarelto (rivaroxaban) works relatively quickly to reach its desired effect and is also eliminated from the body quickly.
Uses
- To reduce the risk of blood clots in people with atrial fibrillation (irregular heartbeat)
- To treat and prevent blood clots in the veins (deep vein thrombosis, DVT) and lungs (pulmonary embolism, PE)
Important information
Before prescribing Xarelto, your doctor will consider the following:
- History of stroke or bleeding in the brain
- Liver or kidney problems
- A medical condition that increases the risk of bleeding
Xarelto can increase the risk of bleeding. This medication does not cause bleeding, but it can make bleeding worse than normal.
How to take Xarelto
Take the tablet with a half glass of water at the same time each day, morning and evening.
If you have difficulty swallowing, you can crush the tablet in cold water or applesauce. Take it immediately.
Dosage
- For atrial fibrillation: 1 tablet once daily
- For blood clots: 2 tablets twice daily for the first 21 days, then 1 tablet once daily starting on day 22
Missed dose
If you miss a dose on the same day, take it as soon as you remember.
If it is already the next day, skip the missed dose.
Onset of action
Xarelto starts working within 2-4 hours.
Common side effects and what to do about them
- Increased bruising from bumps or falls. If the bruise is larger than 10 centimeters, contact your doctor or the Thrombosis Service.
- Prolonged bleeding from cuts or nosebleeds. Apply pressure to the bleeding area. If bleeding does not stop, contact the Thrombosis Service. If a nosebleed lasts more than 10 minutes or occurs frequently, contact your doctor or the Thrombosis Service.
- Blood in the urine or stool, or black stools: Seek medical attention immediately. Contact your doctor or the Thrombosis Service to discuss this medication.
- Fatigue, shortness of breath, or worsening heart palpitations: Contact your doctor or the Thrombosis Service. These could be signs of anemia.
- Report any other side effects of this medication to your pharmacist.
Taking other medications
Your pharmacist usually has an overview of your medications. This is important to know if you can take Xarelto with your other medications. Tell your pharmacist if you are taking any other medications, including:
- Medications from other pharmacies
- Over-the-counter medications from drugstores or supermarkets
- Vitamins or herbal supplements
Don’t use Xarelto if you are pregnant or trying to become pregnant. Do not use Xarelto during pregnancy.
